Good news! You can schedule an appointment at Modern Mental Health right here online. Click the “Request Appointment” button below and choose new client. Make sure to check your email once your appointment is scheduled as you will be receiving some paperwork to sign as well as a welcome packet. All documents can be signed electronically, no need to print! You’ll be able to access consents, billing invoices and receipts, create or change appointments AND communicate with us securely about your treatment.

A credit card must be kept on file to cover the cost of late cancellations. If you need to cancel your appointment, we require a 24 hour notice. If you cancel less than 24 hours to your appointment time or do not show up for your appointment you are still responsible for the full fee.
